Online AIM application e-forms
EMC² now has online e-forms for Individual applications, Family Plan applications and Renewal applications plus a Photo Upload form for those who only need to upload a new photo. (The Individual and Renewal forms include photo upload capability too.) Online applications work with any modern web browser on any smartphone or computer.
To get to the online forms, please go to your Facilitator's website and choose "Applications" from the navigation menu on the left side of the page or if you don't know your Facilitator's website go to AIMProgram.com/applications.

Hemi P from California
Hemi my cat is 24 years old blind and was not doing well physically. I took her to the vet and they basically told me she was just old and was probably going to pass soon. Since I am on AIM I decided to put Hemi on. Within 1 month she started eating on her own. Now it has been 7 months and Hemi has gained a few pounds her fur is fluffy again. She is jumping on the bed and meowing for attention. We bought her a water fountain so she could hear the trickling and know where to drink and she loves it. She sort of plays with the water after she drinks. She is like a kitten again. Her sight has not changed but she is happy and healthy coming up on her 25th birthday. I know without a doubt if she wasn't on AIM she would have passed on.
